Patient Privacy

  1. Patient Portal Notice: The patient portal is a secure tool for routine care needs, including reviewing lab results and medical records, requesting prescription refills, managing appointments, completing check-in forms, accessing statements, making payments, and sending non-urgent questions to our office. Please allow up to 72 hours for a response to portal messages or requests. Do not use the portal for urgent or emergency concerns; for urgent concerns, contact your primary care provider, and for a medical emergency, call 911 immediately or go to the nearest emergency department. 

  2. Payment Policy: Payment of all copays and outstanding balances is due at the time of service. If your insurance card lists a copay, payment is required before you are seen by the provider. Patients with an unpaid balance will not be seen or scheduled for future appointments until the balance is paid in full. A $10 monthly late fee will be added to any outstanding balance that remains unpaid 30 days after the first statement is issued. Statements are generated every two weeks, and patients will receive a text message with a direct link to make a payment. If the balance is not paid within 10 days of the text notification, a paper statement will be mailed. If you have questions about your balance, please call our office immediately to help avoid late fees, and contact your insurance company for clarification regarding why the balance was applied. 

  3. Health-Related Forms and Letters, printed medical records request: Medical records may be made available through the patient portal for your review when appropriate. If you request printed medical records, a $25 fee will apply, where permitted by applicable law. Please contact our office if you have questions about a form, letter, or medical-records request. We reserve the right to decline requests to complete health-related forms, paperwork, or letters. Depending on the nature of the request, you may be asked to schedule an appointment to discuss the request with your provider. Our providers do not perform disability examinations and do not complete disability evaluation forms. If a routine form or letter is completed, a $50 fee will be assessed for each request, including FMLA forms and brief paperwork or letters. These fees are billed directly to the patient and are not covered by insurance. Please allow up to 7 business days for completion of forms, paperwork, or letters. 

  4. No Show/ Late cancellation policy for New and Established patients.

    • New Patients: If you do not arrive for your scheduled appointment or cancel with less than 24 hours (one business day) notice, the appointment will be considered a no-show or late cancellation. New patients must pay $100 before rescheduling.  Repeated missed appointments may result in inability to reschedule or discharge from the practice.

    • Established Patients: If you do not arrive for your scheduled appointment or cancel with less than 24 hours (one business day) notice, the appointment will be considered a no-show or late cancellation. A $75 fee will be charged for each no-show or late cancellation.You will not be rescheduled until any applicable fee has been paid. Repeated missed appointments will result in inability to reschedule or discharge from the practice. 

  5. Late Arrival Policy: Please arrive at least 15 minutes before your scheduled appointment time to allow adequate time for check-in, insurance verification, and completion of any necessary paperwork. Patients who arrive 10 minutes after the scheduled appointment time may not be able to be seen, as late arrivals can disrupt the schedule and delay care for other patients. If you arrive more than 15 minutes after your scheduled appointment time, your appointment will need to be rescheduled at the provider’s discretion. If you are unable to be seen because of a late arrival, the appointment may be considered a no-show and subject to the applicable no-show fee. Please call the office as soon as possible if you expect to arrive late. 

  6. Rude or Aggressive Behavior Policy: Our practice is committed to providing a safe, respectful, and professional environment for patients, visitors, and staff. If inappropriate behavior occurs, we will make reasonable efforts to address the concern respectfully and may ask the individual to stop, lower their voice, or leave the office. We ask all patients and visitors to communicate respectfully so that we can provide safe, quality care to everyone. 

  7. Controlled Substance Prescribing policy: Our practice is not a pain management clinic and generally does not prescribe or manage long-term narcotic or other controlled substances. If long-term controlled-substance treatment is needed, it may need to be managed by your primary care provider or a pain management provider. We remain committed to treating the underlying rheumatologic cause of pain whenever possible.
